LifeBio captures life stories for patients in Home Care and Home Health. Our HIPAA-compliant program educates staff and provides opportunities for deeper engagement and more personalized care. The proven reminiscence methods of LifeBio are extremely beneficial for staff, family, and patients. Question templates are provided to help the patient’s life story unfold.

The LifeBio Memory app or a LifeBio Journal can be used for recording the story without delay. 

LifeBio’s questions can be customized to fit into your existing onboarding process for patients. That information can be used to create printed life story books and story summary materials for each patient. Staff and families can assist in the story-gathering process. If necessary, LifeBio can do a phone interview to help gather the information.

The LifeBio Story Team takes the provided information, and creates relevant care planning tools, such as:

• Life Story Book – For the patient to keep 
• Snapshot Summary – To display for others to learn more about the person 
• Action Plan – For staff education behind-the-scenes

LifeBio gives caregivers the opportunity to engage with the patient on a personal level instead of defining them by their medical or daily care issues. Seeing the person’s strengths and goals will be important to reducing loneliness and caring for the whole person.
